Please follow the steps below to arrange your travel vaccinations:
By submitting your travel health forms for review and consultation, you agree to a $70 nurse consultation fee. This fee will be invoiced when your forms are received and is payable regardless of whether you proceed with vaccinations or not.
Click the link below to download our travel form. Fill in as much detail as possible about your travel plans, destinations, and any previous vaccinations.
Once you have completed the form, phone us on (04) 528 3048 to book an appointment with our Travel Nurse. During your consultation, the nurse will review your travel plans and recommend the appropriate vaccinations and health advice.
Please note: There is a travel consultation fee for this service.
You will be asked to email your completed travel form to us before your appointment. This allows our nurse time to research and prepare for your consultation.
Once you are happy with the recommended vaccinations, we will arrange an appointment for the vaccines to be administered.
Vaccines usually take around 7 days to arrive once ordered.
